Assist in identifying root causes of behaviors and consider individualized interventions prior toutilizing medications or restraints unless clinically indicated otherwise.
Promote evidence-based non-pharmacological practices to address behavioral and psychologicalsymptoms of dementia.
Evaluate the effectiveness of individual behavior management interventions utilizing nursing documentation, flow sheets, checklists, etc.
Collaborate with the interdisciplinary team (IDT) to develop individualized resident-centered plansof care.
Ensure effective communication occurs other departments across all shifts.
Assist with bathing, dressing and/or feeding patients based on individual abilities Monitor the resident environment (i.
e.
lounge, dining room, etc.
) for changes that could agitateresidents with mental health disorders Participate in the interdisciplinary meeting and Psychotropic review meeting Coordinate and facilitate programs for residents with mental health disorders that are engaging and appropriate for residents at varying stages of cognitive decline.
Ensure that activities are individualized and customized based on the resident's prior lifestyle(occupation, family, hobbies), preferences and comforts.

Estimated Salary: $20 to $28 per hour based on qualifications.
